Income & finances

  • Recent pay stubs (last 30 days)
  • Federal tax returns (last 2 years)
  • W-2 or 1099 forms
  • Bank statements (last 2 months)
  • Benefits award letters (SSI, SSDI, VA, retirement)

Home ownership (504 programs)

  • Proof of home ownership (deed or property tax record)
  • Mortgage statement (if applicable)
  • Homeowners insurance declaration
  • Utility bill (proof of occupancy)

Repair & disaster

  • Repair estimates from licensed contractors
  • Photos of damage or needed repairs
  • FEMA registration or insurance claim (504 Disaster)
